Authorization
to Proceed (ATP) Form (doc) - Requires a signature
from the Qualified Individual (QI)/Responsible Party (RP)
representative. It is required as the primary authorization
for a spill or drill response from NRC on behalf of the client.
The ATP form is faxed promptly to the QI/RP by the International
Operations Center (IOC) Duty Officer and serves as NRC's authorization
to mobilize response resources. In most cases, NRC will initiate
a response based on verbal authorization, but will require
a signed ATP soon thereafter and will be placed in the NRC
case file.

AMPD
Authorization to Proceed Form (doc) - This form
is used as an authorization for NRC to provide Average Most
Probable Discharge (AMPD) coverage. A locally prepared form
used by the client will be acceptable as long as the required
information is available. Signatures are required. In lieu
of signatures, vessel Master's name will suffice on message
traffic received from ships at sea. This form should be submitted
in accordance with NRC Contract Amendment No. 2, Clause 2.4(b)
of the NRC Services Agreement, and should be provided to the
International Operations Center (IOC) 24 hours prior to commencement
of transfer/lightering operations.

| Initial
Spill Report (pdf) - Aclient may
initiate oil spill or drill response activities for any size
discharge, or threat of such discharge, through NRC's International
Operations Center (IOC). Upon notification, the IOC Duty Officer
will document the initial spill or drill information on the
Initial Spill Report form by asking specific questions of the
caller. |
